Registration

Instructions:

If you are enrolling students in grades K-11 of our parish Faith Formation Program or in the St. Dennis school, use the appropriate faith formation or school registration form. Do not use the form found on this site. Faith Formation and school registration packets can be found in Fellowship Hall and in the parish office.

Parishioner registration forms can be found in Fellowship Hall, in the parish office or can be downloaded from this site.

Completed forms and payment can be returned in three ways:

1. Placed in the collection basket at Mass with "GIFT Registration" written on the envelope.
2. Dropped off at the parish or school office.
3. Mailed to GIFT Registration, 413 Dempsey Road, Madison, WI 53714

Checks should be made out to St. Dennis Parish.
